Types of Hemorrhoids

At Hemorrhoid Institute, we believe that informed patients make the best decisions about their care. Understanding the different types of hemorrhoids and related conditions can help you recognize symptoms and seek the most effective treatment. Below, we outline the key differences between internal hemorrhoids, external hemorrhoids, thrombosed hemorrhoids, and anal fissures.

Internal Hemorrhoids

Internal hemorrhoids develop inside the rectum and are often not visible, unless they are protruding. Because they are located in an area with fewer pain-sensing nerves, they typically cause little to no discomfort. However, they can still lead to symptoms that require medical attention.

Key Characteristics:

  • Painless rectal bleeding
or spotting, often noticed on toilet paper or in the stool
  • Prolapse, called thrombosed internal hemorrhoids (hemorrhoid protruding outside the anus), which may require manual repositioning
  • Mucus discharge and itching around the anal area
  • Can progress from mild to severe if untreated

External Hemorrhoids

External hemorrhoids form under the skin around the anus and are often more symptomatic due to the presence of sensitive nerve endings in the area.

Key Characteristics:

  • Noticeable swelling or lumps near the anus
  • Itching, irritation, or discomfort
  • Pain, especially when sitting or during bowel movements
  • Can become thrombosed, leading to increased pain and swelling

Thrombosed Hemorrhoids

Thrombosed hemorrhoids occur when blood pools in an external hemorrhoid and forms a clot (thrombus). This condition can be extremely painful and may require prompt medical treatment.

Key Characteristics:

  • Sudden, severe pain around the anus
  • A hard, swollen lump that may appear dark or bluish in color
  • Increased discomfort when sitting or moving
  • Possible ulceration if the clot ruptures

Anal Fissures

Anal fissures are small tears or cuts in the lining of the anus, often caused by straining during bowel movements, passing hard stools, or chronic diarrhea. Unlike hemorrhoids, which are swollen veins, fissures result from skin tears and can cause sharp pain.

Key Characteristics:

  • Sharp pain during and after bowel movements
  • Bright red blood on toilet paper or in the stool,
    but not always.
  • A small crack or tear near the anus
  • Possible spasms of the anal sphincter, leading to further discomfort
